Requesting Your Health Care Records


Clients have the right to request copies of their health care records.

To request your records, please contact our office directly using the information below. Requests may require a signed authorization form and will be processed in accordance with Texas law.


How to request records:

  • Submit a written request by email or mail
  • Include your name, contact information, and the records requested
  • Allow reasonable time for processing as permitted by law

Filing a Consumer Complaint


If you wish to file a consumer complaint, you may do so through the Office of the Texas Attorney General – Consumer Protection Division.

Complaints can be submitted online through the Attorney General’s Consumer Protection webpage.
